Your first visit with us
Whether your well-being requires attention to chronic disease or treatment of an acute illness, we want you first visit to be a pleasant experience. New patient registration starts with a phone call to the clinic of your choice. Our relationship begins the minute you call and we get the details needed to start your healthcare journey with us.

How to prepare for your first visit:

Arrive at least 15 minute early for registration and paperwork

Bring your Driver's License or Photo ID

Bring a copy of Immunization Records if applicable

Appointments

Arrive 15 minutes before your scheduled appointment.

Bring your insurance cards to every appointment.

Bring all medications to each appointment.

Copays and deductibles are due at check-in.

Please give a 24-hour notice if you are unable to keep scheduled appointment.

Please note that if you are late for your appointment (>15 minutes) we will do our best to accommodate you, however depending on patient load for the day it may be necessary to reschedule your appointment for a different day or time. While we strive to minimize your wait time, emergencies do sometimes occur and take priority. We appreciate your understanding in these cases.
Insurance and Billing

Patients are responsible for all copays and deductibles are due at the time of service.

Patients are responsible for any services received that may not be covered by your insurance plan resulting in account balances.

We must be able to verify your coverage before services are rendered.

Inform our staff if there are changes in insurance, home address, phone numbers or employment.

To reach our Billing Department regarding your bill or questions regarding insurance coverage please call any of our locations and chose Option 5.
Walk-In Policy
Our goal is to provide quality individualized care in a timely manner. Depending on patient volume at the time of walking in for an appointment, we do reserve the right to request that you come back at a better time and/or date. Please note that walk-ins are seen by the first available Provider, not by request.
Medication Refills
Don't let your medication run out! Please make sure to ask for any medications needed or that you may need before your next visit at each appointment. If you are low or have no further refills please contact your pharmacy so they can notify us electronically. Please allow 24-48 hours for your prescription to be sent to the pharmacy to fill.

Automated Reminders
Our clinics utilize an automated reminder system to remind your of any upcoming appointments for you and your family. Reminders are sent via phone call or text message. The system calls you 3 days prior to your appointment notifying you of the date, time, provider and location.

After Hour Calls
If you are calling after hours with a non-urgent matter you may leave a message on our message system and your call will be returned next business day.

If you have an urgent matter after hours that is not considered a medical emergency you may call 859.813.9010 to speak directly with our on call Provider.
Emergencies
If you have a medical emergency please call 911 or go directly to the nearest emergency room.
